Successful SMTP delivery does not guarantee inbox placement; sender authentication, reputation and message quality still matter.

Check the sending identity
Verify SPF, DKIM and DMARC for the domain, use a consistent From address and inspect the receiving provider's headers when possible. Check whether the sending IP or domain has a poor reputation.
Freehold CMS can send the message correctly while a receiving provider still classifies it as spam.
